At Orthotics Direct, we provide a payment receipt, Proof of Manufacture, which indicates the casting technique and the materials used to fabricate your custom orthotics, and a Biomechanical Assessment Report.
Orthotics Direct offers a shop from home experience, therefore there is no in-person assessment by a Healthcare Practitioner. Our orthotic technicians design your orthotics from a detailed analysis of the casting of your feet, either via a Foam Impression Kit or Digital Cast.

This form is a breakdown of the insurance documentation we provide, a payment receipt, Proof of Manufacture, which indicates the casting technique and the materials used to fabricate your custom orthotics, and a Biomechanical Assessment Report.
Just put your name and email in then click submit. You will receive two emails. The first will give us consent to send you email communications (i.e. Predetermination Form). Once you have subscribed, you will receive a second email with a link to download your form.
Orthotics Direct will provide the documentation to submit a claim with your insurer yourself. All of the documents we provide (Receipt, Biomechanical Report, and Proof of Manufacturer) are shipped with your completed order.
They will also be available to download on our website when you login to your account, after your order has shipped.
